Clinical Integration.

Big changes in health care are ahead. As health care continues to emphasize value-based care delivery and reimbursement, it is more important than ever for health care organizations to strive for enhanced quality and greater value for patients, providers, payers, and communities. This is where UPMC Pinnacle brings in Concert.

What is Clinical Integration?

CREATING A COMPREHENSIVE INITIATIVE

Clinical Integration is a physician-led and patient-centered approach to healthcare management and coordination. It brings together physicians, hospitals, health systems, government payers and commercial payers through shared data, technology and performance benchmarks to deliver higher quality – and more affordable – care. This integration is deemed sufficient by the FTC to allow the Clinically Integrated Network to jointly contract with payers and hospitals to create and share in the “value” created through their dedication to delivering high-quality care and their commitment to enhancing the patient experience, improving quality and reducing costs.

Remain Independent

Concert is a legal arrangement that allows hospitals and physicians to collaborate on quality and efficiency improvement while remaining independent entities.

Negotiate Collectively

Collectively negotiate with commercial health plans, self-funded employers, and government payers for value-based contracts. Maintain your current fee schedules while participating in add-on arrangements that reward the network for high quality, lower cost care.

Quality and Performance Improvement

Collaborate on care improvements — while maintaining autonomy — and depend on the stability, support and resources of a large provider-centric network.

Technology Resources

The key to clinical integration is shared data. Concert will allow for the sharing of data across practice and health system. With a focus on value over volume, Concert will prioritize technologies that enhance the physician and patient experience and remove operational barriers for participating practices.

Why Clinical Integration?

The transition to value-based payment models has put increased pressure on hospitals and health systems to engage physicians more closely in restructuring the delivery of care. Clinical integration provides not only a strategy for alignment and success in today’s environment, but also the ideal platform to prepare for a variety of value-based payment methodologies (e.g. Pay-for-Performance, Shared Savings, etc.) that will make up an increasing portion of physician reimbursement in the future.

Concert provides the space for both quality and performance improvements, allowing for sharing in financial success in value-based care arrangements; need to ensure focus is on quality first and contracting is ancillary
