Concert, An Integrated Health Alliance.
At a high level, the concept of clinical integration (CI) is a physician-led and patient-centered approach to health care management and coordination. It effectively brings together hospitals, health systems, and physicians through shared data, technology, and performance benchmarks to achieve the “triple aim”: improving the quality of care, reducing health care costs, and enhancing the patient experience.
A CIN like Concert places the patient at the center of care and strives to deliver the right care at the right time and by the right provider.
A CIN is a legal arrangement that allows hospitals and physicians to collaborate on quality and efficiency improvement while remaining independent entities, where such activity is reasonably necessary for them to achieve efficiencies that can benefit their customers (patients, health plans, and employers).
CINs offer physicians an opportunity to take a leadership role in shaping the future of healthcare as it shifts from volume-based to value-based care without restricting their participation in other arrangements such as Accountable Care Organizations (ACOs), by bringing different specialties under common incentive and governance structures.
